The hard part is not finding "an app" - there are plenty. The hard part is family adoption. It is easy for everyone to lean on one coordinator because "they have it / know it / will take care of it." That person builds a system alone, then burns out when nobody else uses it.

Start with one lane, not the whole life

Pick either medications or the next few appointments for the first week. Add documents and extra tasks after two people have successfully used Done / Issue or shown up for a visit. Trying to migrate every specialist, pharmacy, and PDF on night one is how projects die.
